Assumption of risk

Martial arts training, including Brazilian Jiu-Jitsu, self-defense and conditioning, is a physical contact activity. It carries a risk of injury, including sprains, strains, fractures and, in rare cases, serious injury.

I understand these risks and choose to participate (or allow my child to participate) voluntarily.

Health and fitness

I confirm the student is in suitable health to train and has no medical condition that makes training unsafe, or that I have told the instructor about any such condition before training.

The student will stop training and tell the instructor if they feel unwell or injured.

Release

To the fullest extent permitted by law, I release Niyyat Martial Arts, its instructors and MACA Masjid from liability for injury or loss arising from ordinary participation in classes, except where caused by gross negligence or willful misconduct.

I consent to basic first aid and, in an emergency, to the student receiving medical care if I or the emergency contact cannot be reached.

Conduct

Students follow the instructor's directions, train with respect and control, and observe the rules of the masjid. The instructor may pause or end a student's participation for unsafe or disrespectful conduct.

Membership and billing

Membership is billed automatically to the card on file at the rate shown at checkout, starting today and repeating on the same date each billing period, until cancelled.

You can update your card or cancel at any time from the “Manage membership” link. Cancellation takes effect at the end of the period already paid for; partial periods are not refunded.

If a payment fails, the card will be retried automatically. Rates may change with at least 30 days' notice.
